What You Need To Register!
Completed Membership Form -1601
Sign in ink (Red or Blue). If player is 18 or will turn 18 during the playing season, the player MUST sign the 1601.

Picture 1" X 1" (Current, No Hats, Sunglasses).
Birth Certificate
If you are a NEW PLAYER to F.C Clovis, you MUST include a COPY (not the original) of your birth certificate.
Registration Fee
Fees Include: CYSA Membership,CYSA Age Appropriate Licensed Coaches, Playing League Expenses, Uniforms (Player keeps), Individual & Team Pictures.
